Please fill out a Release of Information form to request a copy of your records. Be sure to sign and date the document and submit the completed form to our Release of Information Department.

You can submit the form via our Secure Online Portal, *email to roi@nmortho.net, fax to (505) 724-4401, or of course, deliver it in person.

IMPORTANT— Please note that most standard email does not provide a secure means of communication. There is some risk that any protected health information contained in an email may be disclosed to, or intercepted by, unauthorized third parties. New Mexico Orthopaedics highly encourages the use of our Secure Online Portal.

Please allow 48-72 hours from the time of receipt for our staff to process your request. A valid ID is required to pick up your records.
